Class Location

We are located next to Wellstar Windy Hill Hospital (we are in the building to the left of the hospital if you are looking right at the front of the hospital).

Please use the link above to GPS to Wellstar and then follow the signs to our location. 

Forms: It’s very important that you complete all forms no later than noon on the WEDNESDAY before your scheduled class.

The following forms must be completed by Wednesday before your class. Please click the link below, then read and sign each form.

You will be prompted to enter your name and email address before you access the forms. When completed, we will receive your digitally signed forms via email. There are 4 forms total – Medical History, Authorization, Laser Consent and Certificate Name. 

If you need technical assistance to complete your forms, please contact for help. 

What to Bring: Everything you will need is provided. Please wear comfortable loose-fitting clothes. Please don’t wear makeup or deodorant if you would like treatments on your face and or under arms. Please shave 1-2 days before for hair reduction treatment. 

Hotel Reservations: There are a number of hotels close to the training center. If you need help finding a suitable hotel, please let us know.

Cancellation Policy:
If you cannot attend the date for which you have reserved, you must contact us at least 3 days in advance to reschedule for a later date. With proper notice, you may reschedule once up to 2 years. Once your seat has been reserved there are no refunds issued.

Paying your class balance: If there is a tuition balance on your account, it must be paid by the end of the business day on Friday before your class. If you have questions about paying your tuition balance, please contact Pam Hodges at or by phone at 888-473-1121
